UWEMA Quality Care provides homemaker services that allows the person with a disability to receive support with household tasks necessary for the persons to remain healthy and safe in their home. Basic Homemaker services consist of the performance of basic household tasks within the person’s home

Homemaker - Enhanced
UWEMA Quality Care provides homemaker services that allows the person with a disability to receive support with household tasks necessary for the person to remain healthy and safe in their home. Enhanced Homemaker Services consist of the same household tasks as described under Basic Homemaker Services with the addition of either habilitation or extraordinary cleaning provided by a qualified homemaker.

Mentorship Services
UWEMA Quality Care delivers Mentorship services that provide opportunities for persons with intellectual and developmental disabilities to build self-advocacy skills.
Mentorship will support individuals to become more knowledgeable and confident self- advocates. Mentorship will allow individuals to develop and exercise skills necessary to make informed decisions and build independence.

Prevocational Services
UWEMA Quality Care delivers Prevocational Services that provide opportunities for persons with intellectual and developmental disabilities to learn and develop social, interpersonal, time management, and other job-related skills. Prevocational Services are designed to assist a person with intellectual and developmental disabilities who is interested in finding and securing competitive employment in the future.

Supported Community Connections
UWEMA Quality Care will provide SCC services to individuals with intellectual and developmental disabilities in the community. SCC services provide individuals with opportunities to build relationships and natural supports in their community, continue to interact with members of the community, promote safety, security, and independence, and work on personal goals. UWEMA Quality Care will provide services through typical community settings.

Individual Residential Services and Supports
UWEMA Quality Care will provide 24/7 IRSS services to individuals with intellectual and WU developmental disabilities interested in living independently in the community of their choice.

Enhanced Homemaker Services must involve extraordinary/deep cleaning or include the following minimum habilitative standards:

o Step-by-step instructions to complete household activities o Strategies to develop/maintain skills

o Cueing/Prompts provided

Mentorship services are provided to persons to promote self-advocacy through methods such as instructing, providing experiences, modeling and advising and include:
• Assistance in interviewing potential providers,
• Assistance in understanding complicated health and safety issues,
• Assistance with participation on private and public boards, advisory groups and
commissions, and

UWEMA Quality Care will ensure that following standards will be implemented when providing Prevocational Services:
• Prevocational Services will be directed to habilitative rather than explicit employment objectives and will be provided in a variety of locations separate from the participant’s private residence or other residential living arrangement;
• Prevocational Services will be designed to increase general employment skills and not primarily directed at teaching job specific skills;
• Persons receiving Prevocational Services will be compensated for work in accordance with applicable federal laws and regulations and at less than fifty (50) percent of the minimum wage. UWEMA Quality Care will comply with Department of Labor Regulations when paying participants less than minimum wage;

SCC learning environment allows individuals to receive services on a one-time basis or recurring basis based on personal needs. Services will be provided on a one-to-one basis or in groups of two (2) to five (5) persons.
UWEMA Quality Care will utilize SCC services to provide individuals with experiences, accessing their community competently, confidently, regularly, and safely; expose individuals to a variety of cultural, social, recreational, volunteer, and employment opportunities; and educate individuals about ways to access their community in an informed manner. SCC services will teach individuals how to plan, choose, and learn while accessing their community and provide opportunities to practice these behaviors.

IRSS will be provided to three (3) or fewer persons receiving services (PRS) living together in a personal care alternative (PCA) setting or Host Home setting, personally designing services to meet the needs of each individual. The individual has the choice to live in a home owned or leased by the agency, their own home/apartment, or a Host Home. Service delivery will be offered through the PCA and Host Home models at UWEMA Quality Care.
